One in four Australians have at least two chronic health conditions1. For these people, our health system can seem un-coordinated. Australia’s Healthcare Homes has initiated the need for clinicians to connect with both their patients and as part of care teams for improved healthcare.

Ocean and Livecare have partnered to provide powered online Shared Care Planning with advanced telemedicine.  This approach allows members of the care team to communicate directly with patients about their care plan, remotely.  Members of the care team communicate securely with each other and can even have case conferences with up to 14 carers including the patient themself.

Ocean and Livecare believe that in combining forces to benefit the Healthcare Homes initiative, clinicians will be better equipped for superior patient care.
